TRAINING ROOM HOURS OF OPERATION
Training room hours are posted on the training room door. The training room will be open at least one half-hour before practice begins, or as early as needed and will remain open at least one half-hour after practice. It is the student-athlete’s responsibility to make every effort to come into the training room for treatment or evaluation in a timely manner after practice and events. A current week’s schedule of practices and events will be posted outside the training room (on the glass of the weight room) and the cell phone numbers of the training staff are listed on the top of this schedule. Treatments and other medical care outside practice or game times can be scheduled with the training staff on a case-by-case basis. When the training room in not in use, the facility will be locked at ALL TIMES. These facilities are under the direct supervision of the head athletic trainer and staff. No one else is to be allowed to use this facility without the knowledge and approval of the head athletic trainer.
No athlete or patient will be allowed in the training room without supervision. All therapeutic modalities must be operated by the athletics training staff due to their potential danger. A list of training room policies is posted in the training room.
TRAINING ROOM MEDICAL RECORDS, PRIVACY, HIPAA COMPLIANCE
Professional organizations and the media, on occasion, will request medical information on specific players from the athletic training staff. Since medical problems are confidential, the athletics training staff will ask each athlete for a Medical Release Authorization as a condition for the release of that information. Information will not be released to the press or to a professional athletics team without a release waiver signed by the student-athlete.
All medical records are retained for 7 years, after which time they are destroyed.
UAH STUDENT HEALTH CLINIC
The University operates a student health clinic which is also open to UAH students taking a least 3 credit hours and is designed to take care of basic health needs. A nurse practitioner may prescribe medications as needed. Treatment for illness, health testing, immunizations, counseling, preventative health services, and vaccinations are examples of services at the health clinic. The clinic is located on the third floor of Wilson Hall room 325 and currently charges a fee for each visit which must be paid at the time of the visit. Additional fees may be charged for testing, lab work, and other services. The health clinic phone number is 256-824-6775. They do not file insurance, but they can give a receipt which can be turned into a student’s insurance agency.
UAH COUNSELING CENTER
The University operates a counseling center that is staffed by professional counselors trained to help students. The center is located in Executive Plaza across Sparkman Drive from the Bevill Center. The phone number is 256-824-6203 and they will take appointments. Services at the counseling center are free to students.
