1 NCAA Academic Performance Program DI – The APP is completed by the DoAC and approved by the AD. It is on the NCAA My Applications Portal.
2. NCAA Graduation Success Rates/Academic Success Rates Report – These reports are completed by UAH’s office of institutional research, with support from the DoAC. It is approved by institutional authorities.
3. NCAA Academic Tracking System DII – The DoAC completes and submits the ATS with approval from the AD by required date. It is on the NCAA My Applications Portal.
4. Sports Sponsorship & Demographics Form DII – The SSD form is completed by the DoAC no later than August 15th of the academic year and approved by the AD. It is on the NCAA My Applications portal.
5. Membership Financial Reporting- Completed by the Director of Institutional Finance through the NCAA My Applications portal. It is approved by the President.
6. NCAA Self Study DII- Completed every five years using the NCAA My Applications Portal. Various administrators have taken point on this report and it is approved by the AD.
7. NCAA Squad/Financial Aid Listings – See Financial Aid Section V. NCAA Compliance Assistant for Internet software is used for these reports.
8. NCAA Eligibility Lists – See Eligibility Section III. NCAA Compliance Assistant for Internet software is used for these reports.
9. FormsDirect Software – This internet software use is a work in progress. So far reports from the sports medicine department and some Buckley Amendment waivers and financial aid disclosures have been used. Plans are to use this software for forms
10.EADA Reports – Are completed by the Director of Institutional Finance, with assistance from the DoAC. They are approved by the university administration.
11. NCAA Waiver Requests- The DoAC will take the lead on NCAA bylaw waiver requests and use the NCAA My Applications portal. The President, AD or FAR my initiate waiver requests as they deem fit and they have access to the portal. Typically the DoAC will write and submit the waiver requests, with appropriate information collected from interested parties. All requests will have the appropriate release and authorization documents to request the waiver as mandated by the NCAA through the portal. Results will be distributed to the appropriate parties but at no time will the results be copied and disseminated, and at no time will they be furnished other than verbally or in a separate written memo/letter to a PSA/SA/parent or outside party. Appeals will be processed in the same manner and the process will start all over.
12. Sexual Violence Attestation Form- Due March 15th through the NCAA Apps and signed by the President, AD, and Title IX Coordinator.
13. NCAA Attestation Forms of Compliance for President and AD- The UAH President and AD take and pass their assigned modules through the NCAA Learning Portal and attest to NCAA rules and regulations compliance annually.
